Form Name Description / Purpose Format
Membership Application Use this form if you would like to apply for membership in LJAA.
Membership Renewal If you are already an LJAA member, use this form to renew your membership. This form is for Active and Sustaining Members only. If you are an Associate Member and want to renew your membership, use the Associate Member Renewal form instead.
Associate Member Renewal Associate Members are those who significantly contribute to LJAA, but who do not exhibit their works in our gallery or on our web site.
Consignment Form We must have a signed copy of this form if you want to sell works of art through La Jolla Art Association venues.
Artworks List This is the inventory list that must be completed at the start of every show. It automatically becomes part of the Consignment Agreement with LJAA.
Exhibition Standards & Gallery Rules Before each show, there is a group pre-show meeting. This form must be read, completed, signed, and submitted at that meeting.
Web Update Package This form should accompany each Web Update Package, along with a check covering the $10 fee for each update package.
Expense Reimbursement If you incur expenses that are to be reimbursed by LJAA, send this form, along with receipts and a stamped, self-addressed envelope to our Treasurer.
Group Show Press Release Our Public Relations Director now takes care of all press releases for Group Shows. Please use this form to send the relevant information.
